Blondie considers herself a real life doll. She has had 5 breast augmentations (first one at age 18), liposuction, cheeks and constant botox. She is currently 1700 cc's which is 7.5 pounds all together. Blondie doesn't currently work but she does have "doll fans" that pay for her lifestyle, which apparently means another increase. Blondie is 200% overfilled and already showing signs of her body rejecting the implants. The Doctors send her to a mammogram immediately but her thoughts are, "dolls don't get breast cancer", "I just want to look like tits on a stick".

Elaine is 45 and her nose has collapsed. She was 12 when her sister elbowed her in the nose. Later in life she was in a car accident and fractured it again. When she went into surgery to help her sinus problems, she came out with a hole in her nose. Her goal with the Doctors is to love herself again after 24 years of hating. Dr. Nassif's original plan is to use hanging tissue from her forehead and attach it to the side of her nose, like an elephant trunk. To make sure it stays alive, it would have to be there for 6 weeks before the next step. Right before surgery, Elaine opts not to use forehead tissue, making Dr. Nassif's job more difficult. His new plan is to use harvested rib to reconstruct the shape and use skin from the other side of her nose to fill in the hole. During surgery, however, he discovers that her skin is unusable. He's giving her a 50% improvement and hoping she is satisfied. He is always open to the forehead flap surgery in the future. But Elaine LOVES her new appearance and doesn't regret a thing.

Kayden feels that she has mutant boobs. Her parents were evangelical christians and she was an effeminate boy. They would perform exorcisms on her to expel the feminine aspects causing her to pretend to enjoy "boy" things. In her 20's she decided to start her transition. First she started with hormone therapy and then went for breast augmentation. She asked for 350 cc's and woke up with 700 cc's. Three months later she experienced burning in her cleavage because her muscle has moved away from the sternum due to the large size of her new breasts. His response to her displeasure was "You're a transgender, you want to look like a doll... you should thank me". Dr. Dubrow finds that her implant is wider than her breast cavity and acknowledges that there are difficulties with a male chest. Within 72 hours of a breast augmentation the body forms a capsule of scar tissue. Dr. Dubrow is using that scar tissue as a new wall for Kayden's breasts implants. At her post-op appointment Kayden discloses that she is disappointment in her breast shape... wanting more contouring. Dr. Dubrow blames himself for letting her expect things that she shouldn't. She doesn't have the same tissue as a woman so her expectations have to be altered.
